Safety is the most important issue in the workplace. We must make a conscious decision to work safely and act responsibly to protect others and ourselves.
Unfortunately accidents do happen. When one occurs, an accident report should be completed. The information in the report is used to protect both employees and employers. To make sure that the information is accurate the accident report should be completed as soon as practically possible while we still remember the facts of the accident.
Lets look at a sample of an accident report.

Firstly we need to record the date and the time when the accident happened, the name of the person who's injured, the place where the accident happened, the details of the accident, and the treatment.
We also need to note if the accident will be subject to, or covered by, any insurance claims.
Finally, we need to sign and write down our names as reporters.
